Your First Appointment .
Your first appointment at LifeWeigh Bariatrics will be filled with valuable information about how you can prepare for weight loss surgery. You will meet with the nurse, program coordinator, dietitian, exercise physiologist and one of the surgeons, Dr. Allen T. Mikhail or Dr. Jeffery Rosen. This initial appointment is approximately 3 hours long.
Prior to your first appointment, please complete the following:
1. Please call you insurance company to ensure you have coverage for weight loss surgery. We will assist you with the insurance process and our office will call your insurance company during your first visit to obtain detailed information about your coverage for weight loss surgery.
We encourage all patients to read their insurance policy handbook and to become familiar with their coverage and pre-certification requirements. We participate with many medical insurance plans. If we are a participating provider on your plan, we will accept your valid insurance card for the services we provide you. Your co-payment will be required at the time of service. Any deductible applied to our fee or non-covered charges will be your responsibility. We will file a claim on your behalf to your insurance company. We do require a current copy of your insurance card and a picture ID with each visit. If you do not have your card with you, we will ask that you pay for the visit. Ask: "Does my policy cover weight loss surgery for morbid obesity?”
If yes, ask “what are the specific criteria that are required for approval?”
Please make a note of this specific criteria.
If no, contact the Human Resources department of the employer that provides
your health insurance.
Prior to your initial appointment, you must mail or fax your insurance card along with your completed health history packet (see step #3 below).
Work with your primary care physician to obtain a referral for a consultation with a specialist. Your primary care physician will help you to determine if your policy covers weight loss surgery. Please fill out the health history form above and return to our office prior to your initial appointment. Along with your health history form, please also send a copy of your driver's license and insurance card. If you have an HMO, you must also mail or fax your referral before your appointment.

4. It is mandatory to view our First Visit Movie Presentation located in the link below. You will take a review quiz after. Please call our office for a password to our quiz and video. You will only view this if you have made an initial appointment with one of our offices.
